Hi guys just wondering if anyone can help me. I am currently doing a post graduate diploma in District Nursing and am struggling with my last assignment. I've got to do a written paper in which i explore case management in Primary Care. I have then got to offer a workable strategy to develop the case management of patients, families and or groups within my area of work. Any ideas would be greatly appreciated. Due in in a week!!!!
