Default Patient teaching and disease prevention

Do you think that new medical apps can bridge the learning curve between practitioners and patients to increase their knowledge and encourage them to take an active role in their disease prevention?

I think if medical applications are used to teach patients about particular disease processes they will feel more empowered and knowledgeable. If they understand what is happening they may be able to take an active role in changing their health patterns.
