I have a trick an simple question.

Do I need to fill out incident report if I found clustered pustules on a finger, which family member said that it looks like her mother used to have on her face. We been treating,I believe, the ending stage of that on her face, which looks like a sligtly dry skin now.

I saw it , called the doctor ,received order for treatment,fiil out phone order, started brown sheet( TX sheet)because it had to be done twicw a day,got the medicine,started Tx. ...
...And my manager did write me up for not filling out incident report...

I don't get it. Did your boss explain why she wanted you to fill out an incident report?
